Reliant Rehabilitation is seeking a licensed Physical Therapist to deliver comprehensive therapy services in skilled nursing facilities. You will evaluate patients, develop and monitor treatment plans, and coordinate discharge planning while collaborating with facility teams.

Qualifications

• Graduate of an APTA-accredited school of physical therapy.
• State license as physical therapist in good standing.
• Strong skills in clinical documentation and patient interaction.

Responsibilities

• Provide physical therapy and related services to patients in SNF settings.
• Develop initial and ongoing treatment plans of care and discharge plans.
• Supervise PTAs and Rehab Techs; coordinate with facility staff.
• Maintain compliance with state and federal regulations and infection control procedures.
