Job summary

Greater Health Now seeks a Clinical Program Specialist to drive the development and expansion of EPC and MIH initiatives across the GHN region. You will coordinate partnerships with health systems, EMS, clinics, and community groups to advance integrated, patient-centered care.

The role emphasizes collaboration, stakeholder engagement, and data-informed program improvement, with responsibility for planning regional events, tracking milestones, and reporting outcomes.

Qualifications

• Minimum of three (3) years of experience in healthcare systems, clinical program coordination, population health, or related fields.
• Experience working with interdisciplinary teams and community partnerships.
• Knowledge of MIH, EPC, and care transformation initiatives is preferred.

Responsibilities

• Support development and growth of GHN clinical initiatives including EPC and MIH.
• Identify and pursue opportunities to expand clinical partnerships and recruit stakeholders.
• Coordinate program activities, timelines, and stakeholder communications.
• Facilitate meetings, learning collaboratives, and regional events.
• Track metrics and support quality improvement and outcome reporting.
