Job summary

Procare Integrated Health and Transport seeks a senior MIH program leader to manage day-to-day operations, risk, and performance across hospital partnerships. The role includes direct MIH patient encounters, clinician supervision, and ensuring compliance with Medical Director-approved protocols.

The ideal candidate will launch the UMMC MIH program on a tight timeline, coordinate cross-functional teams, and maintain robust program metrics and reporting for renewal and expansion.

Qualifications

• Current Maryland paramedic licensure in good standing (NREMT-P/NRP preferred).
• Current Maryland RN licensure with equivalent home-based or transitional care clinical experience will be considered.
• BLS and ACLS certification current at hire.
• Valid driver's license with acceptable driving record and background checks.

Responsibilities

• Owns MIH referrals from Epic order-set receipt through episode closure and follow-ups.
• Leads launch and steady-state execution of the UMMC MIH program starting 1 Nov 2026.
• Maintains a live referral tracker and reports volume against targets.
• Ensures scheduling within physician-ordered windows and documents reasons for delays.
• Coordinates program logistics: labs, medications, equipment, and follow-upCare.
• Manages program rhythm: daily referrals, weekly huddle, and partner meetings.
• Escalates compliance risks and maintains HIPAA-compliant records.
• Supervises MIH Clinicians: scheduling, coaching, and performance reviews.
