The role emphasizes interprofessional rounds, evidence-based care, and collaboration with physicians, nurses, and case managers to optimize patient outcomes and access.

Qualifications

• Has PharmD or BS in Pharmacy.
• Completion of ASHP-accredited PGY1 residency or 3 years related experience.
• Licensed or eligible for Texas licensure within 90 days of hire.

Responsibilities

• Assess and optimize medication regimens for clinical outcomes.
• Monitor therapy and adverse events; adjust per protocol.
• Collaborate with physicians, nurses and case managers for coordinated care.
• Educate patients on medications, side effects, and adherence.
• Participate in patient rounds, quality initiatives, and pathways development.
• Support medication-use evaluations and regulatory compliance.
