Job summary

UTMB Health seeks a Clinical Practice Pharmacist specializing in Transplant, Hematology, and Oncology to provide advanced medication therapy management across inpatient and ambulatory settings.

Responsibilities include leading evidence-based care, collaborating with physicians and nursing teams, and educating patients to optimize outcomes and safety. This role supports quality initiatives and regulatory compliance within a dynamic academic health system.

Qualifications

• Pharmacist licensed in Texas or eligible for licensure.
• Must be licensed with the Texas State Board of Pharmacy within 90 days of hire.
• PharmD or BS/pharmacist degree required.
• Completion of ASHP PGY1 residency or 3 years of related experience.

Responsibilities

• Assess medication regimens, optimize clinical outcomes and cost-effectiveness.
• Monitor therapeutic responses and adverse events, adjusting therapy per protocol.
• Implement evidence-based guidelines and internal policies for safe therapy.
• Collaborate with physicians, nurses, and case managers for patient-centered care.
• Participate in rounds, case conferences, and interdepartmental meetings.
• Serve as a clinical resource for drug information and decision-making.
• Support medication-use processes with operational staff to ensure safety and efficiency.
• Educate patients on medications, adherence, and potential side effects.
