Job summary

Qualifications

• Bachelor of Science in Pharmacy or Doctor of Pharmacy degree.
• Completion of ASHP-accredited PGY1 residency or 3 years related experience.
• Licensed to practice pharmacy in Texas or eligible for licensure with license within 90 days.

Responsibilities

• Assess regimens, resolve therapy problems, and optimize outcomes and cost.
• Monitor responses and adverse events, adjust therapy per protocol with prescribers.
• Implement evidence-based guidelines and regulatory requirements for safe, cost-effective therapy.
• Collaborate with physicians, nurses, and other professionals for patient-centered care.
• Participate in rounds, case conferences, and interdepartmental meetings.
• Provide drug information and evidence-based recommendations to colleagues.
• Support medication-use processes with staff to ensure safe distribution.
• Identify and report medication safety events and drive improvements.
• Educate patients on medications, proper use, and adherence.
• Precept pharmacy learners and participate in experiential education.
• Develop and refine clinical pathways and collaborative practice agreements.
