Job summary

Advocate Health is seeking a licensed Physical Therapist to evaluate patients, develop and implement care plans, and collaborate with physicians to ensure comprehensive rehabilitation. You will tailor modalities, exercises, and education to each patient’s diagnosis, age, and discharge needs, documenting progress and coordinating schedules.

The role requires a Bachelor's degree in Physical Therapy, an applicable state license, and BLS.

Qualifications

• Bachelor's degree in Physical Therapy.
• Applicable state license for Physical Therapy.
• BLS certification as per policy guidelines.

Responsibilities

• Evaluate patients and establish care plans.
• Gather and interpret data from medical records, physician reports, patient and family.
• Establish functional goals with patient and provide evidence-based interventions.
• Document evaluations, tests, progress notes, and outcomes.
• Communicate progress to physicians and other staff; coordinate schedules and discharge planning.
• Provide supervision of PTA's and Rehab Tech/Aide as required.
