Job summary

Advocate Health is seeking a Physical Therapist to evaluate patients, develop individualized treatment plans, and collaborate with a multidisciplinary team in a dynamic clinical setting.

The position requires a Bachelor's Degree in Physical Therapy, applicable state licensure, and CPR/BLS certification. The therapist will educate patients and families, document progress, and participate in discharge planning within a fast-paced environment.

Qualifications

• Bachelor's Degree in PT is required.
• Applicable state licensure as a PT is required.

Responsibilities

• Evaluates patients and establishes a patient care plan including modalities, procedures, exercise programs and education.
• Gathers and interprets data from medical records and physician reports to determine needs and utilize appropriate assistive devices.
• Documents evaluations, tests, progress notes, daily treatment notes, education, outcomes and discharge planning.
• Re-evaluates patients as needed and adjusts goals and treatment.
• Communicates progress to physicians and other staff and coordinates scheduling and discharge planning.
• Provides patient and family education, including home exercise programs and equipment use.
• Directs and supervises PT aides, technicians, students, and volunteers and orients new staff.

Education

Bachelor's Degree in PT from an accredited program
