Job summary

Hireology is seeking a dedicated Physical Therapist (PT) to join our in-house team. You will assess patients, develop individualized treatment plans, and administer PT services to improve mobility and quality of life.

Work within an established plan of care and collaborate with a multidisciplinary team to deliver exceptional patient outcomes. You will need an active Texas PT license, and SNF or long-term care experience is preferred.

Qualifications

• Texas Physical Therapist license in good standing
• SNF or long-term care experience preferred
• Knowledge of physical therapy treatment practices
• Familiar with Net Health Electronic Documentation System preferred
• Ability to manage patients with varying needs and personalities

Responsibilities

• Consult with patients to assess condition and functional limits
• Evaluate assessments and test results
• Develop individualized treatment plans and rehabilitation programs
• Deliver PT treatments to relieve pain and restore function
• Document thoroughly per plan of care and regulations
• Collaborate with interdisciplinary team for high-quality care
• Provide patient and caregiver education and support
• Attend required meetings with Rehab Director
