Job summary

Creative Solutions In HealthCare in Texas is seeking a dedicated Physical Therapist to join our in-house rehab team. You will assess patients, design individualized treatment plans, and administer therapy to improve mobility and quality of life.

The ideal candidate holds an active Texas PT license, has SNF or long-term care experience, is knowledgeable in Net Health documentation, and demonstrates strong communication and collaboration skills.

Qualifications

• Active and valid Texas Physical Therapist license.
• SNF or long-term care experience preferred.
• Current knowledge of physical therapy treatment practices.
• Experience with Net Health Electronic Documentation System preferred.

Responsibilities

• Consult with patients to assess physical condition and functional limitations.
• Develop individualized treatment plans using therapy techniques.
• Administer prescribed PT treatments to relieve pain and restore function.
• Collaborate with interdisciplinary team to ensure high-quality care.
• Complete documentation accurately and timely per policies.
