Job summary

Creative Solutions in Healthcare, Inc. seeks a dedicated Full-Time Physical Therapist (PT) to join our in-house rehab team. You will assess patients, develop individualized treatment plans, and provide therapy to improve mobility, strength, and overall quality of life in a skilled nursing or long-term care setting.

Applicant must hold a current Texas Physical Therapist license. SNF or long-term care experience is preferred, with knowledge of Net Health documentation.

Qualifications

• Active and valid Texas Physical Therapist license.
• In good standing with regulatory agencies and licensing boards.

Responsibilities

• Consult with patients to assess their physical condition and functional limitations.
• Evaluate and interpret patient assessments and test results.
• Develop individualized treatment plans using a variety of therapy techniques.
• Create personalized rehabilitation programs tailored to each patient’s goals.
• Administer medically prescribed physical therapy treatments to relieve pain, restore function, and promote healing.
• Monitor patient response to treatment and modify approaches with the interdisciplinary team.
• Provide clinical support and education to patients, family members, and caregivers.
• Collaborate with interdisciplinary team members to ensure high-quality patient care.
• Complete documentation in accordance with policies and state/federal regulations.
