Job summary

Creative Solutions In HealthCare is seeking a dedicated Physical Therapist (PT) to join our in-house team. You will assess patients, develop individualized treatment plans, and administer therapies to improve mobility, strength, and quality of life.

This full‑time role offers flexibility and a patient-centered approach in a skilled nursing facility setting. Candidates must hold an active Texas PT license, have strong communication, and be capable of collaborating with a multidisciplinary team to

Qualifications

• Active and valid Texas Physical Therapist license.
• SNF or long-term care experience preferred.
• Knowledge of current physical therapy treatment practices.
• Experience with Net Health Electronic Documentation System preferred.

Responsibilities

• Consult with patients to assess physical condition and functional limitations.
• Evaluate and interpret patient assessments and test results.
• Develop individualized treatment plans using various therapy techniques.
• Create personalized rehabilitation programs tailored to goals.
• Administer prescribed physical therapy treatments to relieve pain and restore function.
• Monitor patient response and modify approaches with the care team.
• Complete documentation in accordance with policies and regulations.
• Collaborate with interdisciplinary team to ensure high-quality care.
