Job summary

Hireology is seeking a dedicated Physical Therapist (PT) to join our in-house rehab team in Denton, Texas. This full-time role focuses on improving residents’ mobility, strength, and overall quality of life through person-centered care.

You will assess patients, develop individualized treatment plans, administer PT treatments, and document outcomes in line with the plan of care. Strong collaboration with the interdisciplinary team is essential.

Qualifications

• Active Texas PT license in good standing.
• SNF/long-term care experience preferred.
• Knowledge of current physical therapy practices.
• Experience with Net Health EDS preferred.

Responsibilities

• Assess patients' physical condition and functional limitations.
• Develop and implement individualized treatment plans and rehab programs.
• Administer prescribed PT treatments and monitor progress.
• Document care thoroughly and timely according to policies.
• Collaborate with interdisciplinary team to ensure high-quality care.
• Attend required rehab department meetings.
