MyCare is seeking a Clinical Operations Pharmacy Technician to support primary care clinics and patients, driving prescription drug optimization and medication adherence. The role manages the lifecycle of chronic medications, targeting >80% panel adherence, with outreach before run-out dates and EMR documentation within 24 hours.

Qualifications

• High School Diploma or equivalent.
• Excellent communication skills both written and verbal.
• Applied knowledge of HEDIS, medication adherence, insurance processing, customer service or call center processes and practices.
• Strong analytical skills with the ability to interpret PDC reports and prioritize patient lists based on 'impact ability'.
• Familiarity with Medicare Advantage Star Ratings and the impact of Triple-Weighted HEDIS measures (Diabetes, Hypertension, Cholesterol).

Responsibilities

• Data Tracking (PDC gap lists) and varied administrative, operational, customer support assignments.
• Make outbound and take inbound calls.
• Patient outreach to MyCare Medical members.
• Collect medication history information and identify barriers to adherence.
• Ensure prescriptions are refilled and picked up timely; synchronize medications.
• Communicate with pharmacies regarding refills.
