Job summary

Northwestern Medicine is seeking a Clinical Pharmacist PNE to join our Pharmacy Department in Lake Forest, IL. The role emphasizes safe and effective medication use across hospital settings, coordinating with physicians, nurses, and patients to optimize therapy.

Responsibilities include interpreting orders, monitoring drug interactions, counseling patients and staff, and maintaining formulary and patient profiles. The ideal candidate holds IL pharmacist license and PharmD or BS with license.

Qualifications

• Active Clinical Pharmacist license in the state of Illinois (IDFPR).
• Bachelor’s Degree in Pharmacy or a Doctor of Pharmacy Degree.

Responsibilities

• Interprets medication orders to ensure safety and proper use of medications and coordinates solutions with pharmacy, nursing, and prescribers.
• Monitors medication therapy for drug–drug, drug–diet, or lab interaction concerns and intervenes when needed.
• Evaluates and documents that medications are appropriate for therapeutic outcomes and communicates issues to prescribers.
• Participates in pharmacokinetics and adverse reaction discussions with physicians and staff, adjusting therapy per results.
• Processes all medication order information into the Electronic Health Record to ensure accurate records.
