Qualifications

  • Active Clinical Pharmacist license in the state of Illinois (IDFPR).
  • Bachelor’s Degree in Pharmacy or a Doctor of Pharmacy Degree.

Responsibilities

  • Interprets medication orders to ensure safety and proper use of medications and coordinates with pharmacy, nursing, and prescribers.
  • Monitors medication therapy for interactions and intervenes as needed.
  • Evaluates and documents that medications are appropriate for therapeutic outcomes.
  • Acts as a consultant in pharmacokinetics and adverse reactions to healthcare professionals.
  • Processes medication orders into the Electronic Health Record for accurate dispensing and charging.
  • Maintains comprehensive pharmacy patient profiles with pertinent data affecting therapy decisions.
  • Oversees packaging, labeling, and distribution of medications and controls inventories.
  • Compounds and dispenses pharmaceuticals including sterile products and parenteral nutrition.
  • Verifies orders before delivery and resolves discrepancies.
  • Reviews laboratory data for medication use and recommends changes when needed.
  • Develops and presents education programs for staff and students.
  • Educates patients and families about medications.
  • Responds to emergencies to assist with medication management.
  • Maintains hospital formulary and ensures formulary compliance.
  • Documents clinical activities and collects data as required.
  • Communicates knowledge effectively to patients and professionals.
  • Participates in quality improvement and medication use reviews.
  • Plans workload and oversees staff to achieve department objectives.
  • Reviews restocking ADCs and resolves problems.
  • Ensures hazardous waste procedures are followed.
  • Maintains proficiency through professional development.
  • All other duties as assigned.

Bachelor's Degree in Pharmacy
