Job summary

Northwestern Medicine Central DuPage Hospital in Wheaton, IL is seeking a Clinical Pharmacist PNE to ensure safe and effective medication use. You will interpret orders, monitor therapies, and collaborate with physicians, nurses, and pharmacists to optimize patient outcomes.

You will maintain the formulary, provide education to staff and patients, and participate in quality improvement activities while adhering to regulatory and ethical standards.

Qualifications

• Active Clinical Pharmacist license in the state of Illinois (IDFPR).
• Bachelor’s Degree in Pharmacy or Doctor of Pharmacy.

Responsibilities

• Interprets medication orders to ensure safety and proper use of medications.
• Monitors medication therapy for significant interactions and intervenes when needed.
• Evaluates and documents that the medication(s) are appropriate for achieving desired therapeutic outcomes.
• Educates patients and families about specific medications.
• Maintains the hospital formulary and ensures accurate documentation.
