Job summary

Qualifications

• Bachelor’s Degree in Pharmacy or a Doctor of Pharmacy Degree is required.
• Active Clinical Pharmacist license in Illinois (IDFPR) is required.
• Preferred: completion of a pharmacy residency program or equivalent hospital experience.

Responsibilities

• Interprets medication orders to ensure safe and proper use; coordinates solutions with pharmacy, nursing and prescribers.
• Monitors medication therapy for interactions and therapeutic duplications; intervenes when needed.
• Evaluates medication appropriateness and communicates potential problems to prescribers.
• Acts as a consultant in pharmacokinetics and adverse reactions; participates in therapeutic drug monitoring.
• Processes medication order information into the Electronic Health Record; maintains patient profiles.
• Oversees packaging, labeling, and distribution of medications; manages controlled inventories.
• Compounds and dispenses pharmaceuticals including sterile products and parenteral nutrition.
• Verifies medication orders before delivery; resolves discrepancies.
• Reviews laboratory data for appropriateness; recommends changes when needed.
• Develops and presents orientation and training programs for staff and students.
