Job summary

Advocate Health is seeking a pharmacist to oversee pharmaceutical care, educate patients and caregivers, and design therapy regimens based on biomedical literature. The ideal candidate will have a pharmacy degree (ACPE) or NABP FPGEC, state licensure or reciprocity eligibility, and oncology or infusion experience is preferred.

Qualifications

• Pharmacy degree from ACPE-accredited school or NABP FPGEC certificate.
• Licensed in the state of practice or eligible for reciprocity.
• Oncology, hospital or infusion experience preferred.

Responsibilities

• Oversees pharmaceutical care and assesses literature to design therapy regimens.
• Counsels patients and caregivers on medication use, adverse effects, and adherence.
• Designs and monitors pharmacotherapeutic regimens and documents recommendations.
• Reviews and interprets medication orders and prescriptions accurately.
• Participates in medical emergency management when applicable.
• Supports medication safety and quality improvement activities.
