Qualifications

• Pharmacy degree from ACPE-accredited school or NABP FPGEC certificate.
• No prior experience required; PGY-2 Oncology or Hematology preferred.
• Board Certification in Oncology or Pharmacotherapy preferred.
• Excellent communication, organization, and computer skills.

Responsabilités

• Oversees pharmaceutical care, including retrieving and evaluating biomedical literature and designing medication therapy regimens.
• Counsels patients and caregivers on medication therapy, adverse effects, compliance, and administration.
• Designs, monitors, and evaluates patient-specific regimens per literature and policies.
• Reviews and interprets medication orders and prescriptions.
• Supports medication safety and quality improvement activities.
• Mentors pharmacy teammates and serves as preceptor as assigned.
• Directs accuracy of pharmacy support team to ensure safe workflows and regulatory compliance.
