Qualifications

  • Bachelor’s or Doctorate degree in pharmacy from ACPE-accredited school or NABP FPGEC certification.
  • Licensed to practice pharmacy in Illinois or eligible for reciprocity.
  • Immunization and non-vaccine injectable certification may be required by practice area.
  • Strong communication and organizational abilities.

Responsibilities

  • Oversees pharmaceutical care, retrieves and evaluates biomedical literature, and designs therapy regimens.
  • Counsels patients and caregivers on medication therapy, effects, compliance, and administration.
  • Designs and monitors patient-specific pharmacotherapeutic regimens based on literature.
  • Reviews and interprets medication orders and prescriptions.
  • Participates in medical emergencies when applicable.
  • Supports medication safety and quality improvement activities; reports events.
  • Performs QA and PI of the medication-use system as appropriate.
  • Collaborates to manage formulary adherence and non-formulary medications.
  • Complies with policies, procedures and regulations.
  • Prepares, dispenses, and oversees storage and handling of medications.
  • Serves as preceptor for pharmacy students, residents, and learners.
  • Mentors and assesses pharmacy teammates, residents, and students.
  • Directs and checks accuracy of pharmacy support team work to ensure safety and efficiency.
  • Demonstrates age-specific care and uses growth/development knowledge.
