Job summary

Advocate Health seeks a Pharmacist to oversee pharmaceutical care, review biomedical literature, and design individualized therapy regimens. You will educate patients and caregivers about medications, monitor therapy, and ensure adherence to policies and regulations.

Collaborating with the healthcare team on formulary management and participating in multidisciplinary rounds, you will also mentor students and staff, contribute to safety and quality improvements, and uphold age-specific care

Qualifications

• Licensed pharmacist in the state(s) of practice or eligible for reciprocity.
• Immunization and non-vaccine injectable certification required based on area of practice.
• Credentialing and privileging required based on area of practice.
• Bachelor’s or Doctorate degree in pharmacy from an ACPE-accredited school of pharmacy or FPGEC/NABP.
• No experience required.

Responsibilities

• Oversees pharmaceutical care, including retrieving and evaluating biomedical literature and designing medication therapy regimens.
• Counsels patients and caregivers on medication therapy, adverse effects, compliance, and proper administration.
• Reviews and interprets medication orders and prescriptions.
• Participates in management of medical emergencies when applicable.
• Supports department medication safety and quality improvement activities.
• Collaborates with the healthcare team on formulary adherence and medication-use process management.
• Precepts pharmacy students, residents, and healthcare learners; mentors teammates and ensures safe workflows.
• Ensures care aligned with age-specific needs and department policies.
