Job summary

Advocate Health Care is seeking a Pharmacist to oversee the pharmaceutical care process, evaluate biomedical literature, and design patient-specific therapy regimens. The role includes counseling patients and caregivers on medication therapy, adherence, and safety, as well as reviewing orders and supporting formulary management.

The position emphasizes compliance with policies and regulations, leadership of pharmacy staff, and participation in education/training of learners.

Qualifications

• Pharmacy degree from ACPE-accredited school or NABP FPGEC certificate is required.
• Licensed in the state(s) of practice or eligible for reciprocity.
• Immunization and non-vaccine injectable certification may be required based on area of practice.

Responsibilities

• Oversees pharmaceutical care and designs medication therapy regimens.
• Provides counseling to patients and caregivers on medication therapy and safety.
• Reviews and interprets medication orders and prescriptions.
• Participates in medical emergencies as needed.
• Supports medication safety and quality improvement activities.
