Advocate Health is seeking a licensed pharmacist to oversee comprehensive pharmaceutical care within our clinical setting. You will design patient‑specific regimens, counsel patients and caregivers, and ensure accurate interpretation of orders.

The role includes collaboration with the healthcare team, participation in emergencies, and leading educational activities for students and staff. You will help maintain formulary adherence, support medication safety initiatives, and ensure compliance

Qualifications

• Licensed in the state(s) of practice or eligible for reciprocity.
• Immunization and non‑vaccine injectable certification required.
• Credentialing and privileging required.
• Bachelor’s or Doctorate degree in pharmacy (ACPE‑accredited) or NABP FPGEC certificate.
• No experience required.

Responsabilités

• Oversees pharmaceutical care and designs medication therapy regimens.
• Counsels patients and caregivers on medication therapy and adverse effects.
• Reviews and interprets medication orders and prescriptions.
• Participates in medical emergencies and medication safety activities.
• Maintains formulary adherence and medication-use processes.
• Prepares, dispenses, and handles medications according to regulations.
• Serves as a preceptor for students and residents.
• Directs and checks workflow to ensure regulatory compliance.
• Provides age-appropriate care and supports life-span needs.
