Job summary

Advocate Health Care seeks a licensed pharmacist to lead pharmaceutical care, evaluate literature, and design medication therapy regimens. You will educate patients and caregivers on therapy, safety, adherence, and proper use, while ensuring compliant dispensing and documentation.

You will collaborate with the health care team to ensure formulary adherence, participate in quality improvement, and mentor students and teammates in a fast-paced clinical setting.

Qualifications

• Licensed in state(s) of practice or eligible for reciprocity.
• Immunization and non-vaccine injectable certification required by area.
• Credentialing and privileging required by area.

Responsibilities

• Oversees pharmaceutical care and designs medication regimens.
• Interprets medication orders and prepares dispensing activities.
• Counsels patients and caregivers on therapy, safety and adherence.
• Participates in medication safety and quality-improvement activities.
• Collaborates with healthcare team on formulary adherence.
• Precepts students, residents and staff as assigned.
• Ensures storage, handling and documentation meet regulations.
• Demonstrates growth, development, and team leadership within the department.

Education

Bachelor’s or Doctorate degree in pharmacy
