Job summary

Advocate Health is seeking a licensed pharmacist to oversee pharmaceutical care, evaluate literature, and design patient-specific therapy regimens. You will counsel patients and caregivers on medication use, adverse effects, and adherence, ensuring safe and effective therapy.

The role involves monitoring regimens, participating in emergencies, supporting medication safety initiatives, and collaborating with the health-team to manage formulary adherence and medication-use processes.

Qualifications

• Licensed pharmacist with pharmacy degree from an ACPE-accredited school or NABP FPGEC credential.
• Eligible for licensure in practicing states; immunization/injectable certification may be required based on area.
• Strong communication and organizational abilities; proficient with pharmacy software and systems.

Responsibilities

• Oversee pharmaceutical care, including evaluating literature and designing therapy regimens.
• Counsel patients and caregivers on medication therapy, adverse effects, compliance, and administration.
• Monitor and evaluate patient-specific regimens; ensure compliance with policies and regulations.
• Review medication orders accurately and participate in medical emergency management as needed.
• Support medication safety and quality improvement activities and report events through proper channels.
• Collaborate with the care team on formulary adherence and medication-use processes.
