PharMerica in Warwick, RI, is hiring a full-time Pharmacist for our non-retail long-term care team. You’ll evaluate orders, provide clinical recommendations, and maintain resident Medication profiles.

Qualifications

• Bachelor’s Degree in Pharmacy; Pharm D preferred.
• One year experience in long term care desired.
• Current and valid pharmacist license in state of practice.
• Knowledge of drug therapy guidelines, medication safety and regulatory standards (CMS, DEA, FDA).
• Proficiency in electronic medical records and pharmacy information systems.
• Strong clinical judgement and written/verbal communication.

Responsibilities

• Evaluate physician medication orders for drug appropriateness, correct dosage, potential drug interactions, and proper route of administration.
• Provide clinical recommendations to physicians and healthcare staff regarding medication therapy changes and optimizations.
• Maintain up-to-date patient/resident profiles, including medical history, lab results, and current medications.
• Ensure laboratory tests required for monitoring high-risk medications are appropriately ordered and communicated.
• Collaborate with physicians, nurses, and other healthcare professionals for clinical and technical support.
• Document all communications and interventions in the patient’s medical record.
• Implement and enforce policies and procedures for safe handling, dispensing, and management of medications.
• Ensure compliance with state and federal regulatory requirements related to pharmacy practice.
• Report medication-related errors and adverse drug events to the Pharmacy Manager.
• Educate and support clinical team and healthcare agency staff on safe medication handling procedures.
• Perform other pharmacy-related tasks as assigned.
