PharMerica in Warwick, RI is seeking a full-time Pharmacist for our closed-door, non-retail pharmacy serving long-term care and senior living patients. You will play a key clinical role, ensuring safe medication practices and collaboration with the care team.

Qualifications

• Bachelor’s degree in Pharmacy required; PharmD preferred.
• One year of experience in long-term care desired.
• Current and valid pharmacist license in the state of practice.
• Knowledge of CMS/DEA/FDA guidelines and medication safety.

Responsibilities

• Evaluate physician medication orders for drug appropriateness and dosage.
• Provide clinical recommendations to physicians and healthcare staff.
• Maintain up-to-date patient/resident profiles including meds and labs.
• Ensure monitoring for high-risk medications and communicate with the healthcare team.
• Collaborate with physicians, nurses and other professionals to provide support.
• Document communications and interventions in the medical record.
• Enforce safe handling, dispensing and oversight of medications; ensure regulatory compliance.
• Report medication-related errors to Pharmacy Manager.
