Qualifications

• Bachelor's Degree in Pharmacy and license in the state.
• Pharm D preferred and long-term care experience desired.
• Knowledge of CMS/DEA/FDA guidelines and regulatory standards.
• Proficiency in electronic medical records and pharmacy systems.
• Strong clinical judgement and clear written and verbal communication.

Responsibilities

• Evaluate physician medication orders for appropriateness, dosage, interactions.
• Provide clinical recommendations to physicians and staff on therapy changes.
• Maintain patient/resident profiles with medical history and meds.
• Ensure monitoring labs for high-risk meds are ordered and reviewed.
• Collaborate with physicians, nurses and other professionals for support.
• Document communications and interventions in the medical record.
• Enforce policies for safe handling, dispensing and oversight of meds.
• Comply with state and federal pharmacy regulations and reporting.
