PharMerica, an affiliate of BrightSpring Health Services, is seeking a full-time Pharmacist to join our Concord, NH closed-door team. You will review orders for safety and efficacy, collaborate with physicians and care teams, and maintain comprehensive resident drug profiles in a non-retail setting.

The role emphasizes regulatory compliance, patient safety, and ongoing professional development within a growing national network.

Qualifications

• Bachelor’s Degree in Pharmacy required.
• One year experience in long term care desired.
• Current and valid pharmacist license in the state of practice.
• Knowledge of drug therapy guidelines, medication safety practices, and regulatory standards (CMS, DEA, FDA).
• Proficiency in electronic medical records and pharmacy information systems.
• Strong clinical judgement and communication skills.

Responsibilities

• Evaluate physician medication orders for drug appropriateness, dosage, interactions, and administration.
• Provide clinical recommendations to physicians and staff regarding therapy changes.
• Maintain patient/resident profiles with medical history, labs, and medications.
• Ensure monitoring laboratory tests for high-risk meds are ordered and reviewed.
• Collaborate with physicians, nurses and other health care professionals.
• Document communications and interventions in medical records.
