PharMerica is seeking a full-time Pharmacist in Wilkes-Barre, PA to join our non-retail, long-term care pharmacy team. You’ll review orders, provide clinical recommendations, and maintain patient profiles across a national network focused on high-quality pharmaceutical care.

Qualifications

• Pharmacist license in the state of practice is required.
• Bachelor’s Degree in Pharmacy with PharmD preferred.
• Experience in long-term care is desirable.
• Strong knowledge of drug therapy guidelines and regulatory standards (CMS, DEA, FDA).
• Proficiency with EMR and pharmacy information systems.
• Excellent written and verbal communication and clinical judgement.

Responsibilities

• Evaluate physician medication orders for appropriateness, dosage, interactions, and administration route.
• Provide clinical recommendations to physicians and staff on medication therapy changes.
• Maintain patient/resident profiles with medical history, labs, and medications.
• Ensure monitoring labs for high‑risk meds are ordered and reviewed.
• Collaborate with physicians and nurses for clinical and technical support.
• Document all interventions in the medical record and communicate with families/caregivers.
• Enforce policies for safe handling, dispensing, and management of medications.
• Ensure compliance with state and federal pharmacy regulations.
• Report medication errors and adverse events to Pharmacy Manager.
• Educate clinical staff on safe medication handling procedures.
