Pharmacy Alternatives in Christiansburg, VA is hiring a full-time Pharmacist for a non-retail, closed-door pharmacy serving long-term care and senior living clients. You will evaluate orders, provide clinical recommendations, and maintain patient profiles in a collaborative team setting.

Formación

• Bachelor’s degree in Pharmacy required; PharmD preferred.
• One year of long-term care experience desired.
• Current and valid pharmacist license in the state of practice.
• Knowledge of drug therapy guidelines, medication safety practices, and regulatory standards.
• Proficiency in electronic medical records and pharmacy information systems.
• Strong clinical judgement and effective written and verbal communication.

Responsabilidades

• Evaluate physician medication orders for drug appropriateness, dosage, interactions, and administration route.
• Provide clinical recommendations to physicians and healthcare staff on therapy changes.
• Maintain up-to-date patient/resident profiles including medical history, labs, and current medications.
• Ensure lab tests for monitoring high-risk meds are ordered, reviewed, and communicated to the healthcare team.
• Collaborate with physicians, nurses, and other professionals for clinical and technical support.
• Document communications and clinical interventions in the patient’s medical record.
