Pharmacy Alternatives, an affiliate of PharMerica, seeks a Pharmacist in Redding, CA for a full-time, non-retail position serving long-term care and senior living clients. You'll evaluate orders, collaborate with care teams, and ensure patient safety through proper dispensing and monitoring.

Qualifications

• Bachelor's degree in Pharmacy; PharmD preferred.
• One year experience in long-term care desired.
• Current pharmacist license in the state of practice.
• Knowledge of drug therapy guidelines, medication safety practices, and regulatory standards.

Responsibilities

• Evaluate physician medication orders for appropriateness, dosing, interactions, and administration routes.
• Provide clinical recommendations to physicians and healthcare staff on medication therapy changes.
• Maintain patient/resident profiles with medical history, labs, and current medications.
• Ensure required lab monitoring for high-risk medications is ordered and reviewed.
• Collaborate with physicians, nurses and healthcare professionals to provide support.
• Document communications and interventions in the medical record.
