You will evaluate orders, provide therapy recommendations, and maintain resident profiles while ensuring regulatory compliance and high-quality patient care. The role offers ongoing training, growth, and a team-focused environment.

Qualifications

• Pharmacist license in the state of practice (GA)
• 1 year of long-term care experience preferred
• Pharmacy degree(s) required or equivalent
• Knowledge of drug therapy guidelines and safety practices
• Familiarity with CMS/DEA/FDA regulatory standards
• Proficiency with EMR and pharmacy information systems
• Strong written and verbal communication and clinical judgement

Responsibilities

• Evaluate physician medication orders for appropriateness, dosage, interactions, and administration route.
• Provide clinical recommendations to physicians and healthcare staff.
• Maintain up-to-date patient/resident profiles and medication lists.
• Ensure labs for monitoring high-risk meds are ordered and reviewed.
• Collaborate with physicians, nurses, and other healthcare professionals.
• Document all interventions in the patient record and communicate with families.
• Enforce policies for safe handling, dispensing, and medication management.
• Comply with state and federal pharmacy regulations.
• Report medication-related errors to Pharmacy Manager.
• Educate clinical staff on safe medication handling procedures.
• Perform other pharmacy-related tasks as assigned.
