Qualifications

• Bachelor's Degree in Pharmacy required or PharmD preferred.
• One year experience in long term care desired.
• Current and valid pharmacist license in state of practice.
• Knowledge of CMS/DEA/FDA guidelines and regulatory standards.
• Proficiency with electronic medical records and pharmacy information systems.
• Strong clinical judgement and communication skills.

Responsibilities

• Evaluate physician medication orders for drug appropriateness, dosage, interactions, and administration route.
• Provide clinical recommendations to physicians and healthcare staff on medication therapy changes.
• Maintain up-to-date patient/resident profiles including medical history and current medications.
• Ensure laboratory tests for high-risk medications are ordered and reviewed.
• Collaborate with physicians, nurses, and healthcare staff to provide clinical support.
• Document communications and interventions in patient records including family interactions.
• Enforce policies for safe handling, dispensing, and oversight of medications.
• Ensure compliance with state and federal pharmacy regulations.
• Report medication errors and adverse events to Pharmacy Manager.
