Qualifikationen

Aufgaben

• Evaluate physician medication orders for drug appropriateness and dosing.
• Provide clinical recommendations to physicians and staff on therapy changes.
• Maintain up-to-date patient profiles and review medications.
• Ensure monitoring tests for high-risk meds are ordered and reviewed.
• Collaborate with physicians, nurses and healthcare team for patient care.
• Document interventions in the patient record and communicate with families.
• Enforce safe handling, dispensing and medication management procedures.
• Ensure compliance with state and federal pharmacy regulations.
• Report medication errors and educate the clinical team on procedures.
• Perform other pharmacy-related tasks as assigned.
