Allina Health is seeking a Pharmacist to perform medication therapy management at the Abbott Northwestern Heart Hospital Pharmacy. The primary location will be the ANW Heart Hospital, with work at the MPLS Call Center as needed, and an 8-hour day/evening schedule with occasional weekend rotation.

This 1.0 FTE role emphasizes collaboration with health care professionals to optimize patient care, education for patients and staff, and ongoing quality improvement initiatives.

Qualifications

• Bachelor's degree in Pharmacy or Doctor of Pharmacy (PharmD) as stated.
• Postgraduate year 2 residency experience or similar.

Responsabilités

• Coordinates improvement and quality opportunities using best practices.
• Mentors and promotes staff pharmacists and educates health care teams.
• Evaluates medication use and participates in formulary reviews.
• Coordinates resident and student education and preceptorship.
• Monitors patient safety and contributes to adverse event analyses.
• Leads specialty practice areas and develops new clinical services.
