Allina Health in Minneapolis is seeking a Pharmacist for the Heart Hospital Pharmacy. This 1.0 FTE role offers an 8-hour day/evening shift with occasional weekend rotation, primarily at Allina Health Heart Hospital with work at the ANW campus and MPLS Pharmacy Call Center as needed.
The pharmacist provides optimal pharmaceutical care, reviews medications for safety and efficacy, fills prescriptions, counsels patients, and supports clinical decision-making and staff training while complying with
Qualifications
- Bachelor's degree in Pharmacy or PharmD from an accredited school.
- 0 to 2 years of identified pharmacy experience.
- 2 to 5 years of retail pharmacy experience (preferred).
Responsabilités
- Reviews medications to ensure safe, timely and cost-effective use.
- Evaluates appropriate medical dose, route, frequency and administration; performs quality assurance.
- Counsels patients and caregivers on new medications, refills and OTC products.
- Provides clinical pharmacotherapeutic recommendations and medication therapy management to improve adherence.
- Coaches and trains pharmacy staff; ensures HIPAA and privacy compliance; follows all laws and regulations.
