Job summary

Allina Health is seeking a Pharmacist to provide medication therapy management and education across hospital and clinic settings in Minnesota and Wisconsin. The primary location is the Heart Hospital Pharmacy with occasional work at the ANW campus and MPLS call center.

The role requires PharmD or PharmD-equivalent, MN or WI pharmacist licensure, and a valid driver's license for MN/WI. Expect a day/evening schedule with potential weekend rotation and a competitive hourly pay range.

Qualifications

• Bachelor's degree in Pharmacy or PharmD; PGY-2 residency or equivalent experience.
• Licensed Pharmacist in MN / WI; valid driver's license for MN and WI residents.

Responsibilities

• Coordinate medication therapy management with clinical care teams.
• Educate and mentor patients, pharmacists and providers.
• Lead practice development, guidelines, and training of staff pharmacists.
• Participate in drug formulary reviews and adverse drug event review processes.
