Allina Health in Minneapolis, MN is seeking a pharmacist to lead medication therapy management and collaborate with clinicians to optimize patient care across Heart Hospital Pharmacy. The role emphasizes education, practice guidance, and quality improvement.

You will participate in drug formulary reviews, mentor staff, contribute to research, and support resident and student education while maintaining safety and compliance across primary locations and call center operations.

Qualifications

• Bachelor's degree in Pharmacy or PharmD with PGY-2 or equivalent.
• Resident or equivalent post-graduate training is preferred.
• Licenses required to practice in MN/WI as applicable.

Responsabilités

• Coordinates medication therapy management and collaborates with health care teams.
• Educates patients, pharmacists and clinicians on pharmacotherapy and safety.
• Leads quality improvement efforts and audits for service quality and compliance.
• Participates in drug formulary reviews and dissemination of best practices.
• Mentors staff pharmacists and supports clinical service development.
• Coordinates resident and student education and preceptorship.

Postgraduate year 2 residency experience or similar
