Job summary

UNC Health in Raleigh, NC is seeking a highly skilled clinical pharmacist to support critical care and inpatient cardiology. You will collaborate with a multidisciplinary team to optimize medication therapy, participate in rounds, and ensure patient safety through evidence-based practice.

The role includes teaching pharmacy students and residents, contributing to guidelines, and advancing quality improvement projects.

Qualifications

• In-depth knowledge of pharmacotherapy, pharmacokinetics, and drug-drug interactions.
• Strong understanding of clinical guidelines, evidence-based medicine, and pharmaceutical care models.
• Excellent communication and interpersonal skills to collaborate with healthcare professionals and patients.
• Proficient computer skills for documentation.

Responsibilities

• Review medical charts and assess patients' medication regimens for appropriateness
• Develop and implement individualized treatment plans with dosing adjustments
• Order entry into EHR by reviewing and interpreting orders for accuracy
• Respond to pharmacokinetic consults by assessing appropriateness, planning, and ordering per policies
• Attend teaching of pharmacy students and residents on rotations and PGY-1/PGY-2 residents
• Participate in quality improvement initiatives and medication-related research projects
• Adhere to legal and ethical standards related to pharmacy practice
• Provide patient education and counseling on proper medication use and precautions
