The University of North Carolina Hospitals and Clinics Department of Pharmacy is seeking a Clinical Pharmacist to provide pharmaceutical care to adult heart and vascular inpatients. The role includes pharmacotherapy, medication review, and patient counseling in a multidisciplinary setting.

Hours are 1:30 pm–10:00 pm, Monday through Friday with weekend rotation, onsite at UNC Hospitals in Chapel Hill. New graduates may apply with NC licensure within six months of orientation.

Qualifications

• Pharmacotherapy services and medication profile review.
• Response to drug information questions and order verification.
• Distributive functions and counseling.

Responsabilités

• Receives prescriptions, interprets prescriptions, and reviews the patient's medication profile/history.
• Utilizes patient data to provide optimal drug therapy recommendations and clinical interventions.
• Oversees preparation and dispensing of medications including sterile products.
• Ensures formulary compliance and cost-effective therapeutic substitutions.
• Counsels patients and families and mentors pharmacy staff and students.
