Job summary

UNC Medical Center in Chapel Hill, NC seeks a Clinical Pharmacist to provide comprehensive pharmaceutical care to adult heart and vascular inpatients. The role includes pharmacotherapy services, medication reconciliation, and patient counseling in a multidisciplinary setting.

The position offers evening hours (1:30 pm–10:00 pm) with weekend rotation, and opportunities to precept students as part of UNC Eshelman School of Pharmacy partnerships.

Qualifications

• PharmD or BS Pharmacy Degree required.
• Active pharmacist licensure with NC Board of Pharmacy or eligibility before initiation of employment.
• Board Certified Pharmacotherapy Specialist (BCPS) and residency (PGY1/PGY2) preferred.
• No prior experience required (professional experience requirements).

Responsibilities

• Receives prescriptions, interprets prescriptions, and reviews the patient's medication profile/history; clarifies issues with prescribers and staff.
• Provides pharmacotherapy recommendations and clinical interventions.
• Oversees preparation and dispensing of medications including sterile products.
• Follows formulary and therapeutic substitutions to optimize therapy and control costs.
• Completes documentation in medical records and other required systems.
• Counsels patients and families on pharmaceutical care issues as requested.
• Directs workflow and supervises technicians and students on shift.
• Adheres to policies, procedures, and regulations.
• Identifies and participates in process improvement opportunities.
• Precepts students and residents and trains new staff.
