Job summary

Suvida Healthcare is seeking a Clinical Pharmacist in Tucson to strengthen patient engagement and optimize medication therapy in a Medicare-focused, Hispanic senior program.

You will support clinical staff, provide drug information, and participate in quality initiatives to improve outcomes and cost of care. Bilingual English/Spanish is required, with experience in population health and case management helpful.

Qualifications

• Doctor of Pharmacy degree or equivalent education.
• Active, unrestricted Pharmacy license in Arizona, or ability to obtain one.
• 3+ years of clinical pharmacist experience.
• Bilingual English/Spanish required.

Responsibilities

• Provide patient-centered care to optimize medication therapy and outcomes.
• Perform medication management services and resolve medication-related problems.
• Coordinate medication reconciliation and education across the care continuum.
• Review plans, meds, allergies, labs to address medication-related issues.
• Collaborate with providers and care teams to set disease state goals.
• Support program initiatives and quality improvement efforts.
