Job summary

Suvida Healthcare is seeking a Clinical Pharmacist to strengthen the provider-patient relationship by optimizing medication therapy and reducing costs. You will support the clinical team with medication management, population health initiatives, and chronic disease planning for COPD, DM, and CHF.

You will review plans, counsel patients, collaborate with providers, and participate in quality-improvement programs to enhance care and patient satisfaction across our Hispanic Medicare population.

Qualifications

• Bilingual requirement: English and Spanish.
• Pharmacist degree or equivalent.
• Active license in Arizona or ability to obtain one.
• Several years of clinical pharmacist experience.

Responsibilities

• Provide patient-centered care to optimize medication therapy and improve outcomes.
• Perform medication management to identify/address problems and prevent adverse events.
• Coordinate medication reconciliation across the continuum of care and educate patients.
• Review care plans, medications and labs to resolve drug-related issues.
• Assess prescribed therapy for appropriateness, effectiveness and adherence.
• Document outcomes and follow up with patients/caregivers.

Skills

Bilingual English/Spanish

Education

Pharm D. degree or equivalent education in pharmacy

Active, unrestricted Pharmacy license in Arizons, or the ability to obtain one

3+ years of clinical pharmacist experience
