Job summary

Suvida Healthcare seeks a Clinical Pharmacist to support provider-patient engagement by optimizing medication therapy and integrating clinical programs. The role focuses on medication management, adherence, and case management within a population health framework.

You will collaborate with providers and the care team to develop patient-specific goals, ensure safety and effectiveness of pharmacotherapy, and participate in quality initiatives to enhance outcomes and reduce costs.

Qualifications

• Pharm D. degree or equivalent education in pharmacy.
• Active, unrestricted Pharmacy license in Arizona (AZ).
• 3+ years of clinical pharmacist experience.
• Bilingual in English and Spanish is required.

Responsibilities

• Provide patient centered care to optimize medication therapy and improve outcomes.
• Perform medication management to identify, resolve, and prevent medication-related problems.
• Coordinate medication reconciliation across the continuum of care with education and outcome monitoring.
• Review patient care plans, medications, allergies, labs, and other records to resolve medication-related problems.
• Assess prescribed therapy for appropriate use, dosing, interactions, and adherence.
• Follow-up with patients/caregivers and document outcomes.
