Qualifications

• 3+ years of clinical pharmacist experience
• Bilingual English and Spanish required
• Active, unrestricted Pharmacy license in AZ or ability to obtain one

Responsibilities

• Provide patient-centered care to optimize medication therapy and improve outcomes
• Perform medication management and resolve medication-related problems
• Coordinate medication reconciliation across care continuum and educate patients
• Review patient care plans and monitor outcomes to prevent adverse drug events
• Collaborate with providers and care teams to set disease state therapy goals
• Support pharmacy initiatives and quality improvement programs
