Job summary

Suvida Healthcare seeks a Clinical Pharmacist to strengthen the provider-patient relationship through medication management and program integration. You will support clinical staff, optimize therapy, and improve outcomes while reducing costs.

Emphasizing culture, language, and compassionate care, you will collaborate with a multidisciplinary team to enhance patient experience. You will rely on your Pharm D.

Qualifications

• Pharm D. degree or equivalent education in pharmacy.
• Active, unrestricted Pharmacy license in Arizons, or the ability to obtain one.
• 3+ years of clinical pharmacist experience.
• 3 + years’ experience effectively builds informal relationship networks inside and outside the organization.
• Advanced in Microsoft office platforms including Word, Excel, and PowerPoint.
• Bilingual/Bicultural (English and Spanish) required.

Responsibilities

• Provide patient centered care to optimize medication therapy and improve clinical outcomes.
• Perform medication management services to identify, resolve, and prevent problems.
• Coordinate medication reconciliation and educate patients to prevent readmissions and adverse reactions.
• Review patient care plans, medications, allergies, labs, and other data to resolve issues.
• Assess and adjust prescribed therapy for effectiveness and safety.

Education
