Job summary

The role requires a PharmD, an active AZ license or ability to obtain, and 3+ years of clinical experience with bilingual English/Spanish capabilities. You will work with a multidisciplinary team to improve quality of care and patient

Qualifications

• Pharm D. degree or equivalent in pharmacy.
• Active, unrestricted Pharmacy license in Arizona or the ability to obtain one.
• 3+ years of clinical pharmacist experience.
• 3+ years’ experience building informal networks inside/outside the organization.
• Advanced in Microsoft Office (Word, Excel, PowerPoint).
• Bilingual/Bicultural (English and Spanish) required.

Responsibilities

• Provide patient-centered care to optimize medication therapy and improve outcomes.
• Perform medication management to identify, resolve, and prevent drug-related problems.
• Coordinate medication reconciliation across the continuum of care with education and adherence support.
• Review patient care plans, meds, allergies, labs to resolve medication-related problems.
• Assess prescribed therapy for unnecessary meds, ineffective meds, or adherence concerns.
• Follow up with patients and caregivers and document outcomes.
• Educate patients on chronic disease medications and therapy goals.
• Collaborate with providers and care teams to set disease-state goals.
• Support clinical programs and disease-state management under CPAs.
