Suvida Healthcare is seeking a Clinical Pharmacist to support medication management, clinical program integration, and practice transformation within our primary care model serving Medicare-eligible Hispanic seniors.

You will collaborate with providers and care teams to optimize therapy, improve outcomes, reduce costs, and educate patients, while participating in quality improvement and data-driven initiatives. English and Spanish language skills are required for patient and team communication.

Qualifications

• 3+ years of clinical pharmacist experience.
• 3 + years’ experience effectively builds informal relationship networks inside and outside the organization.
• Advanced in Microsoft Office platforms including Word, Excel, and PowerPoint.
• Bilingual/Bicultural (English and Spanish) required

Responsabilités

• Provide patient-centered care to optimize medication therapy and improve outcomes.
• Coordinate medication reconciliation across the continuum of care.
• Review patient care plans, medications, allergies, labs to resolve issues.
• Collaborate with providers to identify and develop patient-specific disease state goals.
• Provide patient education on chronic disease medications and their use.
• Participate in quality improvement programs driven by clinical needs.

Pharm D. degree or equivalent

Active unrestricted Pharmacy license (Arizona)
